Capped Rental Equipment and Routinely Purchased Items
Capped Rental Equipment (Examples - Wheelchairs, hospital beds, alternating pressure pads, air-fluidized beds, patient lifts, trapeze bars)
The equipment you are receiving will be billed to your insurance as a rental. This item is not available for immediate purchase. Your insurance only makes rental payments for this equipment.
During the rental period, necessary repairs will be taken care of as part of the rental agreement. Please call us in Tampa at 813-879-7552 or in Largo at 727-586-2995 if you are having any issues with your equipment.
Medicare and Medicare Replacement Plans:
· Medicare will pay a monthly rental fee for a period not to exceed 13 months, after which ownership of the equipment will be transferred to the Medicare beneficiary.
· Please notify us immediately if you have a change of address, enter the hospital, a skilled nursing facility, or have any other changes in your information. If you no longer need the equipment, please contact us to arrange for pick up. Medicare will only pay for medical equipment that is medically necessary and is being used in the home.
· If there is a break in service due to admission to a hospital or nursing facility, the billing will stop until you are released. The equipment is still owned by Health Aid Company until all 13 months are billed and paid, even if those months are not consecutive.
· After ownership of the equipment is transferred to the Medicare beneficiary, it is the beneficiary’s responsibility to arrange for any required equipment service or repair.

Routinely Purchased Items: (Examples - Canes, walkers, crutches, commodes, seat lift mechanisms, bed rails, traction equipment)
· Equipment in this category can be purchased or rented; however, the total amount paid for monthly rentals cannot exceed the fee schedule purchase amount.
I received instructions and understand that my insurance defines the equipment that I received as a capped rental or routinely purchased item. For capped rental equipment, I understand that this equipment remains the property of Health Aid Company, Inc. until it is converted to purchase after the rental period concludes.
